Record every session's date, duration, format, and supervisor at the time it happens, and collect supervisor sign-off continuously instead of at the end. The board accepts records it can verify; it does not accept memory.
What a defensible record contains
For every supervision session and every block of clinical experience:
- The date, and the exact duration
- The format: individual or group supervision, direct or indirect clinical work
- Who supervised it, with their license
- Supervisor sign-off, gathered close to when the work happened

The three ways tracking fails
- The end-of-journey reconstruction. Years of sessions rebuilt from calendar scraps and memory. Boards notice, and candidates routinely lose hours they actually worked.
- The unsigned log. A perfect personal record that the supervisor never attested. Sign-off gathered at the end depends on that supervisor still being available, still licensed, and still on good terms.
- The single copy. A notebook, one spreadsheet on one laptop. Lost is lost.
