The board accepts records it can verify. This log captures everything Hawaiʻi expects: when, how long, what format, what category, and who signed. Print a stack, or keep one per month. Get initials at the end of every session, not at the end of your journey.

Reminders while you log
- LCSW candidates: track the individual/group split (at least 60 of 100 individual) and your clinical-hours composition.
- Record the session the day it happens. Reconstructed logs lose hours.
- Keep a second copy somewhere that cannot be lost.
